    Terms and Conditions

    Upon Receipt of Loaned Material(s), the Oro Valley Historical Society (OVHS) agrees to the following terms and conditions:
    1. Immediately report to the LENDER any damage, breakage, or loss of borrowed item(s).
    2. Borrowed item(s) will not be cleaned, repaired, re-matted, remounted, or in any way altered without the expressed written permission of the LENDER.
      • The only exception is in case of emergency. In the event and prior to beginning any treatment, OVHS will make every effort to obtain verbal permission from the LENDER and shall request a written confirmation.
    3. Borrowed item(s) placed on exhibit will be labeled according to data furnished by the LENDER on all appropriate labels, exhibition catalogs, press releases, and related materials.
    4. Borrowed item(s) will not be photographed or reproduced in any form without the expressed written permission of the LENDER.
    5. Borrowed item(s) will not be used for any commercial purpose without the expressed written permission of the LENDER.
    6. Borrowed item(s) shall not be transferred to a third party without the expressed written permission of the LENDER.
    7. Borrowed item(s) will be packed for return shipping in the same manner and means or better than original shipping.
    8. OVHS will not be liable for damage and destruction of any borrowed item(s) resulting from natural or calamitous events, including but not limited to fire, vandalism, theft, war, civil commotion, or any natural or civil disaster beyond its control.
    9. OVHS will examine and take a photograph of the item(s) upon receipt and before returning item(s) to the LENDER.
    The Lender agrees to the following terms and conditions:
    1. Lender is responsible to notify OVHS of any change of address, phone number, or contact person information.
    2. Lender must inform OVHS 30-days in advance of withdrawal/return request of any or all item(s) listed in this agreement.

    Lender Agreement

    By signing this form, I certify that I: 1) Am authorized to enter into this loan agreement, 2) Have read and agree to the terms and conditions stated above, 3) Have reviewed the Oro Valley Historical Society Collections Policy, and 4) Have selected the appropriate insurance coverage for this loan (see Lender Liability in Loan Information section above).
